Evaluating the Effect of PSA Investigations

The Petroleum Safety Authority Norway (PSA) has initiated a project in 2022 to evaluate the effect of its investigations.

Every year, the PSA investigates five to 10 serious incidents. The object is both to identify breaches of the regulations and to contribute to learning lessons from an incident.

The project now under way will provide more detailed answers about the effect of these investigations and what lessons they have provided.

In this context, “effect” is defined as changes to knowledge, motivation, procedures, tools or practice at individual companies and at industry level.

Ten selected
The PSA has chosen DNV to carry out the evaluation, which will be based on 10 selected investigation conducted in recent years.

These are as follows:
Aker BP - Tambar - Investigation into walkway incident
Equinor – Aasta Hansteen – investigation of gas leak
Repsol – Gyda – investigation of an incident involving a dropped object
Equinor – Heimdal – investigation of work accident
Gassco/Equinor – Kårstø – investigation of electrical incident with personal injury
Valaris - Rowan Stavanger - investigation of incident with BOP ram door on Rowan Stavanger
Seadrill – West Bollsta – investigation of incident with dropped riser joint
Equinor – Tjeldbergodden – investigation of turbine breakdown and fire
Equinor – Gullfaks C - investigation of oil spill
KCA Deutag – Askeladden – investigation of incident with dropped object and serious injury
The evaluation will be summed up in a report published on the PSA website, and which will be used by the PSA in its improvement work.
